FOR DANCE PROGRAM CLASSES '10-'11

  • Please note that 3 SIGNED enrollment forms and the $35 annual enrollment fee ($50/family) must be received within 10 business days (via mail, fax or in studio) in order to complete enrollment and guarantee your space!

  • PLEASE PRINT the confirmation page you will receive after submitting this form.  You may pay your enrollment fee immediately by credit card by completing the Method of Payment portion and faxing to us.

  - Non-refundable, must be paid once a year only.

NOTE: The $35 annual, non-refundable enrollment fee per dancer (or $50/family) must be received (via mail, fax or in studio) within 10 business days of submitting this form in order to guarantee your space!

In addition, the 3 SIGNED enrollment forms must be received (via mail, fax or in studio) within 10 business days to complete enrollment.  Any student without these 3 signed forms will not be able to start class until these signed forms are received.

*Unless you have been specifically approved for an Advanced class, please call the dance center to confirm appropriate placement before enrolling in Advanced classes.  You may also wish to confirm placement before enrolling in intermediate or int/adv classes.
